How Co-Management Works

Your patient stays your patient

  • 1 Refer — online form below, or fax demographics + wound history to (949) 544-1544.
  • 2 We verify — insurance authorization and clinical eligibility, usually within 2 business days.
  • 3 We treat — supervised protocol per UHMS indications, typically 30–40 sessions.
  • 4 You stay informed — progress notes at every phase; the patient returns to your care.
