For Physicians: How to Refer a Patient for HBOT

When standard care stalls, hyperbaric oxygen is often the next evidence-based step — and referring takes your staff about two minutes. Here's the complete playbook.

Indications that qualify

  • Diabetic lower-extremity ulcers, Wagner Grade 3+, after 30 days of standard care
  • Delayed radiation injury — soft tissue and osteoradionecrosis
  • Chronic refractory osteomyelitis
  • Compromised skin grafts and flaps
  • Other UHMS-recognized indications — call us to discuss any case

What to send

Patient demographics, insurance information, relevant wound history or imaging, and your working diagnosis with ICD-10 if known. Fax to (949) 544-1544 or use the referral form on our For Physicians page. We handle benefits verification and prior authorization — typically within two business days.

Billing & codes

HBOT is billed under G0277 (per 30-minute interval) with facility code C1300. Your office bills nothing and loses nothing — the patient remains yours.
